Verification Forms
Forms are available in Basecamp (login required). This is where you can view the specific documents that you need to submit to complete your FAFSA Verification.
All forms must be completed, and signed with a stylus or printed and physically signed. We can not take electronic signatures on these forms.
Once completed you will upload them into your Action Item in Workday. We will not accept emailed or faxed forms or other documents. Instructions on how to submit your Federal Identity Verification form are found on the form.
