Changing Your Name at CMC

CMC understands that a person’s name helps define a person’s identity and sense of self and we want people to feel comfortable at CMC.

You can update your legal name and chosen name at anytime during your time at Colorado Mountain College and even as an alumni.

Legal Name

To update your legal name at CMC please complete the Name Change Form and attach the legal documentation indicating your previous name and your new name. Once processed your legal name will be updated across all CMC systems/applications. You can also choose to receive a new username and email address to reflect your updated name by checking the box on the form. Please note that a new username and email can take a few weeks to be processed.

Please ensure you also update your legal name with the Social Security Administration, on your FAFSA and with Selective Service.

Chosen Name

If you entered a chosen first name on your Application to CMC it will be used across most CMC systems and documents - see the drop downs below for more information.

You can add or update your chosen name at CMC using the Name Change Form and no documentation is required. If you entered a chosen first name on your application but want to add a chosen middle name this is also the form you would use.

On the form, enter your legal name and then indicate your chosen first name and chosen middle name (if applicable). Please note that last names can only be updated legally.

You can also choose to receive a new username and email address to reflect your chosen name by checking the box on the form. Please note that a new username and email can take a few weeks to be processed.

Please note that there are some documents and systems where legal name has to be used please see the dropdown below.
